Fees and Insurance

Dr. Milbourne's fee is $220 for one 45-minute individual, family or couples therapy session and intake. Sliding scale is available to financially challenged clients. Except for very brief reports or messages, phone therapy, report writing, or other professional services will be charged at the rate of $275 an hour.  Payment in the form of cash or check is requested at the start of each session.  For a 3% convenience fee, credit cards will be accepted. Please see below this rationale for opting to see a provider outside of your insurance.

A payment schedule may be arranged if you have difficulty affording services as well as a sliding scale.

Insurance - CHANGING in late 2022

After two decades of accepting low reimbursement from insurance, through 2022, Dr. Milbourne will be phasing out his in-network status. With insurance companies reducing reimbursement rates and increasing requirements to work in specific ways with clients, it is no longer feasible for many small business owners to thrive in an insurance-based practice. By reducing caseload, Dr. Milbourne will be able to provide more focused and energized treatment to clients, as well as improve his own work-life balance. Transitioning to a non-insurance-based practice reduces variable reimbursement rates and time-consuming administrative requirements that will allow Dr. Milbourne to prioritize client-focused care, including areas of specialty that include working with veterans, first responders and those facing substance-abuse issues. Another benefit of a non-insurance-based practice is added confidentiality, as insurers routinely request client information to accept and/or deny reimbursement. Through a direct-payment model, the client retains control of all of their personal health information.

Dr. Milbourne has participated in many insurance plans, including Independence Blue Cross/Personal Choice and Keystone Healthplan East, Aetna, Tricare, and Medicare.  Most policies have annual deductibles, copayments, or other limits on benefits.  Read your policy carefully and be aware of what is or is not covered.  You may wish to call the insurance help desk and ask about your benefits. However, by the end of 2022, clients will have to pay in full at time of service and accept reimbursement from your policy’s out of network benefits.

All co-pays and deductibles are requested prior to treatment via cash, check or credit card (please see below).

Missed Appointments

We value your time as well as our own; please help us serve you better by keeping scheduled appointments.  Unless cancelled at least 24 hours in advance our policy is to charge for missed appointments at a rate of $75.

About Ivy Pay

Ivy Pay is a HIPAA-secure, PCI-protected payment service that allows you to pay your therapist by saving a credit, debit, HSA or FSA card on-file. Once you've added your card to Ivy Pay once, charges can be quickly and easily processed. There is a 3% convenience fee for using all cards.

Safety & Security

Ivy uses advanced security systems and data encryption to protect both clients and therapists, as well as safeguard against unauthorized transactions and access to personal or financial information. 

Information is encrypted, stored and protected on secure servers. Industry standard SSL encryption is active on every page of the Ivy Pay system. This is the same encryption technology used by banks and brokerages to safeguard financial information. 

How It Works

1. Your therapist will send you an invite text via Ivy Pay’s secure system - this text will come from our system's phone number: (415) 403-2489.

2. Tap on the link in your invite text to be taken to the secure ‘Add Card’ form.

3. Scroll down on the ‘Add Card’ form, add your card details, and tap ‘Save & Pay.’ Your card will be encrypted and stored on our secure servers, and will be charged for that day's session.

4. Moving forward, you will receive a text confirmation any time your therapist uses Ivy Pay to charge your card on-file. This will appear in the same thread as your original invite text. (Tip: if you ever need to update your card details, just tap on the invite link and follow Steps 1-3 above.)
